All forms should be completed and returned prior to the start of care with the exception of the Health Assessment. This form needs to be completed by the 3rd week of admittance. If you choose to not immunize your child, a written statement must be given at enrollment. Different exemptions are allowed. Your reason can be a religious reason; exemption due to a strong personal objection to immunize, or a medical condition in which it is against a doctor’s order to immunize. An enrollment interview is also mandatory.
Each fall, Ummie’S completes an audit of enrollment records. At the conclusion of this audit, families will be notified if anything needs to be updated. Some forms must be updated every year. The Emergency Contact and Agreement Form must be updated every 6 months.
Other records must be updated throughout the year, such as physicals and immunization records. When visiting your child’s physician for a yearly “well-child” appointment, please request a copy of your child’s physical and most recent immunization record. You may bring these items in yourself.
In addition, any time a family’s information changes such as address, place of employment or health insurance provider, a new Emergency Contact and Medical Consent form must be completed.
